Job description

We are seeking a skilled Nurse Practitioner to join our team. The ideal candidate will have a strong background in primary care and a commitment to delivering compassionate care to patients. This position offers the opportunity to work collaboratively with physicians, nurses, and other healthcare professionals to provide comprehensive care to our patients.
Responsibilities:
- Conduct comprehensive physical examination, well woman health assessments, etc.
- Diagnose and treat common acute and chronic illnesses
- Order and interpret diagnostic tests and labs
- Develop and implement treatment plans
- Prescribe appropriate medication
- Provide patient education and counseling on health issues and preventive care
- Collaborate with physicians and other healthcare professionals to ensure coordinated care
- Maintain accurate and up-to-date medical records
- Close all charts, finish all notes, and follow up with all results in a timely manner
- Participate in quality improvement initiatives and other practice activities
- Performs other related duties as assigned
- Adhere to evidenced based and best practice guidelines
- Maintain and participate in all patient centered medical home standards and requirements
- Maintain the highest professional standards in a manner consistent with current office policy
- Follow all current office policies and procedures
Required:
- Current, unrestricted Nurse Practitioner license in NJ
- 2 or more years’ experience in Nursing or relevant area
- Strong clinical skills and knowledge of primary care principles
- Excellent communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
- Commitment to providing high-quality, patient-centered care
Preferred:
- Experience in a Primary Care setting
- Proficient in EPIC
