We are actively seeking a Full-Time Wound Care Nurse Practitioner to serve patients in long-term care facilities.
Role and Responsibilities:
- Provide wound care for long-term care patients in skilled nursing facilities.
- Prescribe appropriate medications and coordinate treatments.
- Communicate regularly with nursing home staff to discuss changes in condition and care plans.
- Collaborate with primary care physicians.
- Assess and monitor residents regularly.
- Assist staff in assessing wound and pressure ulcer risks.
- Evaluate various wound-related factors, including Nutrition, Mobility, Incontinence-associated dermatitis, Pressure ulcers, Diabetic ulcers, Arterial ulcers, Venous ulcers, and Common dermatologic conditions.
Management:
- Implement measures to decrease the risk of wounds with staff recommendations.
- Optimize healing through protocols, including superficial wound debridement, dressing selection, placement of negative pressure wound dressings, and post-myocutaneous flap surgery care.
Qualifications and Education Requirements:
- Current, unrestricted NP license in the state of Ohio.
- Board Certification and DEA licensure.
- Clinical background in wound care.
- Experience in a long-term care setting.
- Ability to work with an EMR.
- Proficiency in managing patients with advanced illness and multiple chronic conditions.
- Ability to work within an interdisciplinary care team to coordinate patient care.

Medical Specialty: Geriatrics
Physical Setting: Long Term Care Facility
Experience:
- Clinical background in adult or geriatric specialties: 1 year (Required)
- Work in a long-term care setting: 1 year (Preferred)
License/Certification:
- Current, unrestricted NP license in the state of Ohio (Required)
- WCC or WCON Certification (Preferred)
- DEA Licensure (Required)
- Board Certification (Required)
