We are actively seeking a Full-Time Wound Care Nurse Practitioner to serve patients in long-term care facilities.

Role and Responsibilities:

• Provide wound care for long-term care patients in skilled nursing facilities.
• Prescribe appropriate medications and coordinate treatments.
• Communicate regularly with nursing home staff to discuss changes in condition and care plans.
• Collaborate with primary care physicians.
• Assess and monitor residents regularly.
• Assist staff in assessing wound and pressure ulcer risks.
• Evaluate various wound-related factors, including Nutrition, Mobility, Incontinence-associated dermatitis, Pressure ulcers, Diabetic ulcers, Arterial ulcers, Venous ulcers, and Common dermatologic conditions.

Management:

• Implement measures to decrease the risk of wounds with staff recommendations.
• Optimize healing through protocols, including superficial wound debridement, dressing selection, placement of negative pressure wound dressings, and post-myocutaneous flap surgery care.

Qualifications and Education Requirements:

• Current, unrestricted NP license in the state of Ohio.
• Board Certification and DEA licensure.
• Clinical background in wound care.
• Experience in a long-term care setting.
• Ability to work with an EMR.
• Proficiency in managing patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Physical Setting: Long Term Care Facility

Experience:

• Clinical background in adult or geriatric specialties: 1 year (Required)
• Work in a long-term care setting: 1 year (Preferred)

License/Certification:

• Current, unrestricted NP license in the state of Ohio (Required)
• WCC or WCON Certification (Preferred)
• DEA Licensure (Required)
• Board Certification (Required)
