Job description

The  Clinical Program Manager role with our Hamot Health Foundation is an exciting full-time opportunity which operates from Monday to Friday, between 8:00 AM and 4:30 PM . The Clinical Program Manager plays a crucial role in community outreach and health education. Their responsibilities include:

  1. Needs Assessment : Identifying and addressing medical, behavioral, and social needs and barriers to care within the community.

  2. Care Planning : Developing comprehensive care plans that bridge gaps in preventive care, address barriers, and support self-management of chronic illnesses, all aligned with clinical standards.

  3. Collaboration : Working closely with community agencies, providers, resources, and caregivers to enhance support for community members.

  4. Community Engagement : Engaging in face-to-face interactions across various community settings, including hospitals, providers’ offices, homes, and other healthcare facilities.

Responsibilities :

The Clinical Program Manager conducts thorough face-to-face assessments encompassing medical, behavioral, pharmacy, and social aspects for community members. By reviewing community health data, they pinpoint gaps in care according to established clinical standards. Their responsibilities include:

  1. Connecting At-Risk Residents : The manager links vulnerable individuals to community agencies, resources, and programs that address healthcare needs, food access, housing stability, behavioral health, and other social determinants of health.

  2. Referring to Appropriate Programs : Based on the Erie/UPMC Hamot community assessment data, they refer community members to suitable case management, health management, or lifestyle programs.

  3. Engaging in Education and Self-Management : The manager actively involves community members in various educational and self-management programs.

  4. Enhancing Knowledge and Skills : They provide community members with relevant educational materials and resources to improve their understanding and abilities related to health and lifestyle management.

The  Clinical Program Manager orchestrates outreach efforts and community events to actively engage community members. Their goal is to collaboratively create  individualized care plans with primary care providers that promote healthy lifestyles, bridge gaps in care, and reduce unnecessary emergency room (ER) visits and hospital readmissions. Key responsibilities include:

  1. Care Plan Coordination : The manager collaborates with individuals, caregivers, primary care providers (PCPs), specialists, community resources, and behavioral health providers to modify and coordinate care plans effectively.

  2. Referrals to Career Programs : They refer community members to clinical career programs, addressing employment needs for UPMC Hamot and its affiliates. This provides valuable opportunities for residents seeking employment and career paths.

  3. Barriers to Health in Underserved Communities : Aligned with the mission of Hamot Health Foundation and its subsidiary, Build Community Development Corporation, the manager identifies and tackles health barriers faced by residents in underserved communities.

  4. Community Outreach and Trust Building : The manager actively participates in community outreach, collaborating with the existing Hamot Health Foundation model in partnership with UPMC For You. They build trust within the community and serve as patient advocates when necessary.

  5. Data-Driven Decision-Making : By developing and maintaining detailed care plans, documenting outreach efforts, tracking contacts, and reporting results, the manager uses data to inform the development and evaluation of clinical programs. These programs aim to close gaps in preventive care, address barriers to care, and support self-management of chronic illnesses.
