- Pay Rate: Negotiable (based on experience)
- Position Status: Full-time, 37.5 hours per week
- Schedule: Monday – Friday, 9:00 am – 5:00 pm *with occasional weekends and evenings

Here’s what to expect on the job:
- Accept the initial report of incidents and prepares investigatory questions based upon the information presented within the initial incident report.
- Identify other tasks that need to be completed to implement a thorough investigation.
- Establish timelines for the process, including steps to be taken and a projected completion date.
- Interview the initial individual who reports an incident, witnesses, potential witnesses, and other persons identified as potentially having knowledge of the reported incident.
- Identify and complete additional interviews deemed necessary from knowledge gathered during the initial investigation.
- Review witness statements, documentary evidence, demonstrative evidence, physical evidence, and testimonial evidence.
- Analyze and evaluate all information to establish facts and assess credibility and to answer the developed investigatory question.
- Submit investigation summaries into EIM (Enterprise Incident Management) system within 1-2 days of completion of the administrative review and within 30 days of the assignment.
- Complete the investigation packet in its entirety.
- Comply with all laws and regulations pertaining to privacy.
- Anticipate and manage “public relations” related concerns and challenges in accordance with all standard operating procedures.
- Pursuant to completion and issuance of the Final Certified Investigation Report, ensure designation of follow-up corrective action steps to appropriate UCP personnel.
- In conjunction with UCP’s Quality and Compliance Manager, monitor the implementation of agreed upon follow-up corrective action steps (i.e. post-investigation reports, interventions for UCP individuals, interventions for UCP staff members, employee discipline proceedings, employee training programs, etc.).
- Participate in both Monthly/Quarterly Incident management meetings to review incidents and identify trends and possible preventative measures with operations staff.
- Participate in quarterly peer reviews of investigations to assess the quality of investigations completed and to provide feedback to investigators for continuous quality improvements.
- Complete individual and Support Service Professional file audits to ensure compliance with all county, regional, and state policies and standards.
Minimum requirements:
- Associate’s Degree or technical certification within a related area preferred.
- 6 months to 2 years of previous job-related experience expected. 2 to 5 years of previous job-related experience preferred.
- Comprehensive knowledge of DD/ID system.
- Knowledge and skill in the application of versatile interviewing techniques.
- Regulatory compliance knowledge specific to the needs of individuals with intellectual and developmental disabilities, autism, and/or the aging population.
Preferred, but not required:
- Regulatory Compliance (ie. Chapter 51, 2380, 6400). Bi-lingual (English/Spanish).
- ODP Certified Investigator Certification preferred.
- If not ODP certified, must obtain certification within 90 days of employment.
- Completion of Quality Management certification preferred.
