Job description

We are seeking a dedicated and compassionate Social Worker to join our team at Midlands Topeka PACE Center.

Summary:
The Social Worker will have the primary responsibility to assess clients' needs, develop treatment plans, and connect them with appropriate resources and services. Through expertise and empathy, the Social Worker will play a crucial role in improving the well-being and quality of life for elderly individuals in our care.

Midland Care’s Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.

For Midland Care PACE participants, PACE means a team of experienced doctors, nurses and other health care professionals getting to know the participant and working with him or her to design a plan to aid in achieving health goals.

Employment Status: Full-Time|Monday – Friday, 8:00 a.m. to 5:00 p.m.

Pay range beginning at $54,000/year - pay based on experience

$3,000 Sign-On Bonus

Qualifications

Required: Current unrestricted State of Kansas license to practice as a Licensed Master Social Worker. Must have knowledge and skills with regard to health care coordination, pain/symptom management, crisis intervention and counseling. Must possess valid Kansas Driver’s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standard as defined by Midland policy.
