The Visiting Nurse Association is seeking an RN to Responsible for coordination and integration of telehealth services into a patient’s plan of care. Captures clinical data key to outcome management. Serves as a clinical resource in cardiac disease management.
Responsibilities
- Management of telehealth monitoring system
- Participate with the oversight committee in the development of patient interventions and be the lead person responsible for ensuring implementation of same, including working with IT in the creation of any/all HCHB documentation that will support the program
- Responsible to participate with Cleveland Clinic in the development and coordination of the hospital heart failure discharge plan and the VNA home health admission plan for each transitioning patient
- Responsible for development and presentation of cardiac training programs for all nurses and therapists.
- Coordinate with medical director to keep him abreast of program development; mutually identify specific topics for a clinical education series to be presented by the medical director
- In collaboration with the marketing department, may speak with medical staff re the benefits of the VNA telehealth program both in Indian River and Brevard counties
- Oversight of patient education related to the program
- Work cooperatively with the QA department in record audits to determine if the program elements are consistently followed and that patient record documentation supports goal achievement.
- To coordinate with clinical managers in the overall care and treatment of patients receiving home health including the benefit of the cardiac program
- Perform patient visits or consultative visits to assist a clinician
- Perform direct supervision of the LPN and any technical support staff as it relates to the cardiac/telehealth program
- Communicate to the Director of Home Health any additional equipment needs, patient outcome reports, results of marketing efforts
- Oversight of process for documenting lab test results for IRC
- Serves as emergency management coordinator for home health
Qualifications
- Current RN license in the State of Florida
- Minimum of 2 year experience in home health
