ChristianaCare is currently recruiting for a full time Hospital Care at Home Clinical Pharmacist, to assist with the coordination and supervision of Hospital Care at Home and to optimize pharmacy practice, pharmacy systems and the medication-use process. Days/hours for this position are Monday through Friday, 10:30 a.m. to 7:00 p.m.
Principle Duties and Responsibilities: - Assist pharmacy management in the advancement of pharmacy operations and Hospital Care at Home - help develop and facilitate the implementation of goals and objectives
- Responsible for effective process design, development, and implementation of Pharmacy operation/clinical projects as assigned and establish the training necessary for each pharmacy user group
- Assume responsibility for the timely dissemination of information related to projects
- Design and implement quality improvement changes to the medication-use system; conduct practice-related projects, clinical research, and/or pharmacy performance improvement initiatives
- Assist in the development and review/modification of polices and/or procedures
- Maintain Pharmacy operations compliance with all State, Federal, and Joint Commission regulations or standards. Maintain department reports and statistics.
Education and Experience Requirements: - BS degree in Pharmacy required
- PharmD degree preferred
- ASHP-accredited PGY-1 Pharmacy Practice Residency required; equivalent experience will be considered (3 years of acute care hospital experience)
- Current/eligible for licensure in the state of Delaware
- Board certification preferred
