TITLE: Home Health RN (Eligible for Sign-on Bonus)
HOME OFFICE: 13770 58th Street Suite 305, Clearwater, FL 33760
HOURS: Monday-Friday, 8:00 AM to 5:00 PM
POSITION SUMMARY:
Reporting to the Clinical Manager, the RN Case Manager is responsible for providing quality care to patients while in they are in their private residences. They are responsible for assessing and providing care, guidance and support for the family within the scope of their credentials..
EDUCATION AND QUALIFICATIONS:
Currently licensed in the State of Florida as a registered nurse
Minimum of one year of nursing experience in skilled home health required
Strong ethical standards and an appropriate professional demeanor
Must possess sound critical thinking, time management, technical skills, analytical ability, and good judgment
The ideal candidate is a well-organized and self-directed individual who is a team player
Must have reliable transportation to perform tasks and responsibilities in a timely and appropriate fashion. Must be able to provide proof of automobile insurance and have a valid Florida driver’s license
ESSENTIAL JOB RESPONSIBILITIES:
Case manages the needs of the patients and families in the field assigned to them.
Collaborate and support plan of care as determined by a physician and healthcare team.
Perform periodic re-evaluation of patients as necessary and make adjustments in treatment plans.
Provide treatment to patients per protocols, utilize OASIS assessment and documentation.
Takes independent action to identify problems
Supports policies and procedures
Functions as a resource person; provides mentoring and guidance for co-workers and additional staff
Maintains positive working relationships with team members of all disciplines
Attends to patient/family complaints and concerns
Creates and promotes initiatives to develop and strengthen relationships with facility partners
Takes a proactive approach to identifying and resolving potential issues
Communicates regularly with the Home Health Care Administrator and continuously assesses areas for risk management
Maintains current with organizational and departmental communication via e-mail. All nursing staff are required to check their e-mail accounts at the start of each shift.
Effectively communicates, collaborates, and coordinates with colleagues and management on issues related to nursing process and service
Creates and implements nursing care plans which are reviewed daily and updated as needed
Considers financial implications in rendering cost-effective nursing care
Ensures professional standards of nursing practice are maintained
Completes required documentation prior to departing from scheduled shift
Collaborates with team physician daily on the status of the patient
Must be able to work rotating on call requirements
Performs other duties as requested by the Clinical Manager
