Job Description
Summary
Oversees the coordination, development, and completion of all MDS Assessments on all units according to facility policy, state, and federal regulations, and following the RAI Manual. Provides overall management/oversight of MDS Coordinators.
- Oversees the coordination, development, and completion of all MDS Assessments on all units according to facility policy, state, and federal regulations, and following the RAI Manual.
- Maintain/update the MDS ARD calendar, adjusting for workload spread and capturing revenue based on patient change in condition/new admission status (capturing CMI increases, IPAs, and clinical data from hospital admission).
- Validate timely completion and signature of MDS assessments by all relevant team members.
- Ensure maintenance and updating of daily PDPM board; run daily PDPM meetings, and delegate these responsibilities during absence.
- Coordinates the development of a written comprehensive plan of care for each resident that identifies the problems/needs of the resident. Indicates the care to be given with measurable objectives and goals to be accomplished.
- Coordinates with Rehab/Coder to establish appropriate diagnosis codes and enters these codes into face sheet.
- Reviews staff documentation to assure that it supports the PDPM categories that resident qualifies for, communicates with nurse managers and physicians to ensure documentation needed for establishing PDPM code is present on chart.
- Participates in daily stand-up/wrap-up, as well as mandatory MDS meetings with corporate Reimbursement Specialist, Triple Check, etc.
- Functions as a resource to the unit by assisting in staff education and in-service training of associates in subject areas related to MDS, Care Planning and resident assessments.
- Reviews/analyze CASPER reports monthly to identify trends and opportunities for improvement and submits report to DON/ADON.
Qualifications
- Minimum of two years of MDS experience
- Specific business experience with older adults or in the retirement housing industry preferred.
- Completion of a Registered Nursing program and currently licensed in the state.
- Current AANAC certification
