13 weeks | Days | 8-hour shift | 40 hours with 37 hours guaranteed weekly

- Utilizing the nursing process, assesses patient/caregiver needs to provide quality care to assigned patients, by evidence-based practice, State and Federal regulations, Vermont State Board of Nursing, and VNAHSR policies and regulations..
- Develop and communicate a realistic plan of care for each patient following evidence-based practice, State and Federal regulations, Vermont State Board of Nursing, and VNAHSR policies and regulations. Ensures that the patient/caregivers are included in the care plan process.
- Serves as Case Manager for assigned patients and actively manages caseloads and visit times following agency productivity standards.
- Effectively fills out and completes OASIS documentation following Agency policies and procedures, and State and Federal regulations, Demonstrates knowledge of and keeps up-to-date on changes to OASIS documentation requirements as needed. Ensures documentation is completed following Agency practice (timeliness.)
- Implements the medical plan of care, nursing interventions, and procedures as necessary to ensure the safe effective quality of care for patients following evidence-based practice, State and Federal regulations, Vermont State Board of Nursing, and VNAHSR policies and regulations.
- Stays up-to-date on a variety of High-Tech skills; IV use and maintenance, wound VAC, complex wound care, etc. Takes action to ensure that infection control standards are maintained throughout the plan of care.
- Following LPN scope of practice, works collaboratively with LPNs to ensure that the plan of care is followed, delegates tasks, and ensures that patient needs are met. Consistently reviews documentation, annotates visit notes, and acts on recommendations following department policies and procedures.
- Evaluates a variety of aspects of the implemented plan of care including, but not limited to identified problems, care provided, and patient's responses, to meet patient/caregiver needs and ensure overall quality of care delivered, following evidence-based practice, State and Federal regulations, Vermont State Board of Nursing and VNAHSR policies and regulations.
- Maintains up-to-date and accurate documentation of nursing care provided to ensure the effective integration of information for use by the health care team to ensure ongoing and continued quality of care, following evidence-based practice, State and Federal regulations, Vermont State Board of Nursing and VNAHSR policies and regulations.
- Follows agency protocol with McKesson processes, including but not limited to, locking-down visits, follow-up notes, visit completion, etc.
- Performs patient and caregiver education to ensure informed patient decisions can be made, including, but not limited to, medication side effects, treatment options, discharge instructions, reportable conditions, ranges of services, etc. following State and Federal regulations, Vermont State Board of Nursing and VNAHSR policies and procedures.
- Appropriately identifies interdisciplinary needs of the patient and documents patient needs accordingly, including, but not limited to physical therapy, social work, etc. Takes action to communicate documented information to other disciplines promptly.
- Accurately and fill out a variety of Agency, State, and Federal forms, including but not limited to, HHABN, Medicare Notice of Non-Coverage, etc. on time. Takes action to ensure that forms are readily available during visits to facilitate timely completion.
- Carries out visits in a time-effective manner. Takes action to ensure that supervisory visits are not combined with routine visits whenever necessary. Takes action to communicate information to the Scheduler on time.
- Assesses patient's needs for HCA services develops and coordinates POC with patient/caregiver, and provides ongoing supervision following State and Federal regulations.
- As necessary, transmit patient incident reports (infections, injuries, etc) to QI specialists on time.
- Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed following department policies and procedures, and State and Federal regulations.
- Coordinates a wide variety of patient care with physicians, providers, team members, vendors, etc. in support of the recommended plan of care to ensure safe and effective patient and family care, following evidence-based practice, Vermont State Board of Nursing and VNAHSR policies and regulations.
- Fulfills agency responsibility to participate in monthly peer review audits. Provides constructive feedback and openly accepts feedback professionally.
- Proactively schedules, coordinates, and facilitates case conferences with the Manager and team members to maximize effective communication with team members. Ensures effective and timely documentation of meetings is maintained.
- Proactively advocates for patient care issues to ensure that overall quality and type of care are sensitive to each specific patient/family's needs.
EXPERIENCE/EDUCATION/QUALIFICATION
Experience:
- At least 2 years of RN HomeCare experience is required
Licenses, Certifications, and/or Registration:
- Must have current and valid State of VT RN licensure
EMR System: McKesson experience preferred
Shift: Monday-Friday 8 am-4 pm some on-call or weekends may be required
