We operate with a network of over 150 therapists throughout Michigan. Were thrilled to invite an exceptional Physical Therapist to join our team in an exceptional subacute rehab facility. Our mission for this program is to make a positive impact on the lives of our patients through compassionate and effective rehabilitation services.

Responsibilities:

• Assess the needs of the geriatric population and develop individualized treatment plans
  
• Implement and monitor therapy programs to improve functional abilities in both returntohome and longterm care settings
  
• Collaborate seamlessly with a multidisciplinary team to provide comprehensive patient care
  
• Provide supervision to Certified Occupational Therapy Assistants (COTAs)
  
• Maintain accurate and uptodate patient records
  
• Educate patients and their families on therapeutic techniques and strategies
  
• Demonstrate a genuine passion for working with older adults
  
• Familiarity with NetHealth software is a plus
