Resp & Qualifications

PURPOSE:
Working as a bridge between the member, clinical and administrative staff, the Navigator helps members, families, and caregivers access the health care delivery system through outreach, follow-up, coordination support, education, communication, and resources. While considered "remote," this is a field-based position where the Community Health Navigator will travel regularly within the Prince George's County area to meet face to face with members, families, providers and other community-based partners in an effort to support members' health needs. Working from a home-base and traveling throughout Prince George's County, the incumbent will also be expected to come into a CareFirst location periodically for meetings, trainings and/or other business-related activities.

ESSENTIAL FUNCTIONS:


QUALIFICATIONS:

Education Level: High School Diploma or GED.

Experience: 3 years community relations/outreach/ customer services/ health educator role.

Preferred Qualifications: Community Health Worker Certification.

Knowledge, Skills and Abilities (KSAs)

Additional Skills and Abilities:

Salary Range: $37,080 - $67,980

Travel Requirements :
Estimate Amount: 50-70% field based.

PHYSICAL DEMANDS:

The associate is primarily seated while performing the duties of the position. Occasional walking or standing is required.
