Position: RN - Home Care - Float- NYC

Our client, a leading healthcare organization in New York City, is seeking a dedicated and experienced RN to join their Home Care team on a full-time contract basis. This is an exciting opportunity to work with a dynamic and supportive team, providing quality care to patients in the comfort of their own homes.

As an RN in the Home Care Float position, you will be responsible for providing skilled nursing care to a diverse patient population across various locations in New York City. You will work closely with the interdisciplinary team to develop and implement individualized care plans, and provide education and support to patients and their families. This is a fast-paced and challenging role that requires flexibility, strong clinical skills, and a passion for providing high-quality care.

Key Responsibilities:

- Conduct initial assessments and ongoing evaluations of patients' medical, physical, and emotional needs

- Develop and implement individualized care plans in collaboration with the interdisciplinary team

- Provide skilled nursing care, including medication administration, wound care, and other treatments as needed

- Educate patients and their families on disease management, medication management, and other aspects of self-care

- Monitor and document patients' progress and communicate any changes or concerns to the interdisciplinary team

- Collaborate with other healthcare professionals to coordinate and ensure continuity of care

- Maintain accurate and timely documentation of all patient care activities

- Adhere to all company policies, procedures, and regulatory requirements

Qualifications:

- Active RN license in the state of New York

- Minimum of 2 years of experience in home care, preferably in a float or per diem capacity

- Strong clinical skills, including medication administration and wound care

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Flexibility to travel to various locations in New York City as needed

- BLS certification

- Knowledge of Medicare and Medicaid regulations is preferred

Home Care experience is not required
- If a candidate doesn't have Home Care experience, they must have 1+ years of MedSurg experience
- These candidates will be doing in home visits, primarily wound care/rehab.
- Minumum 1+ years of overall nursing experience in a related field to be considered for this position
- Will not hire new grads/new nurses with less than 12months of experience
- Extensive 2-4 weeks of orientation
- Monday - Friday schedule 8:30am-4:30pm (must also work one weekend per month)
- 5-7 home visits per day
- Must have a car, a NY issued drivers license, and proper auto insurance
- Parking and other expenses necessary will be reimbursed
