The Oncology Survivorship Advanced Practitioner serves as a comprehensive direct health care provider by functioning in the expanded role of survivorship advanced practitioner. This position helps mitigated barriers survivors may experience in accessing survivorship services and attaining quality of life in the post treatment period. The Oncology Survivorship Advanced Practitioner coordinates and enhances effective communication among survivors, their caregivers and providers for the seamless referral, navigation and coordination of services. The survivorship advanced practitioner focuses on symptom management and refers survivors to appropriate resources to optimize physical function and mental health; remove barriers and facilitate access to services; assist with care coordination; provide general survivor education; and empowers survivors to optimize time with clinicians and seek appropriate care. Serves as a comprehensive direct health care provider by functioning in the expanded role as an advance practitioner by performing acts of medical diagnosis or prescription of medical therapeutic or corrective measures in collaboration with and under the supervision of a physician licensed to practice medicine. On an aggregate level, the Survivorship Advanced Practitioner will track performance of the program in line with service line and network goals and identify opportunities to improve the process of survivorship care. The Survivorship Advanced Practitioner will serve as a liaison between the patient and family, primary care physician, internal and external care providers, specialists, support staff (social workers, genetic counselors, etc.) and the wider healthcare community.
- Leads in developing and establishing an Oncology Survivorship Program in conjunction with the Administrative Director, Oncology Service Line and Chief, Oncology Department.
- Assures the Oncology Survivorship Program meets the requirements of the Commission on Cancer standards.
- Coordinates, collaborates, and works effectively with oncology teams to move survivorship care into all clinical oncology practices.
- Promotes knowledge of Cancer Survivorship across the network, directly to survivors, to specialists, to primary care providers, nursing and in the community.
- Works with Tumor Registry staff to support collection of data and identification of survivorship patients.
- Functions as a patient advocate through the initial survivorship period, serves as a primary point of contact for providers and patients/families regarding survivorship information and resources. Initiates contact with patients and introduces the survivorship program and role at time of treatment end. Provides support to navigate the healthcare system.
- Conducts advanced assessment of the patient’s medical, social, psychosocial and other health care needs and recognizes scope and limitations of the role.
- Sets treatment goals and plans and initiates referrals to hospital and community resources to connect patients with resources and support systems.
- Provides appropriate survivorship teaching, outreach and education to patients and families with the goal of ensuring the patient is empowered to manage his or her own health to include teaching wellness habits and educating patients on the prevention of potential health problems resulting from their cancer treatment.
- Conducts follow-up conversations as needed with patients and communicates concerns, changes or social needs in patient health to appropriate physicians and other care providers.
- Ensures data sets and appropriate data collection is in place to track program outcomes.
- Actively participates in the Cancer Committee including quarterly reporting on the percentage of patients receiving a survivorship care plan and submitting and presenting a comprehensive annual report on the survivorship program/process (monitoring and evaluation of the process, accomplishments, improvements, and planned initiatives).
- Promotes and participates in quality improvement activities for survivors.
- Stays current on the latest oncology developments and participates in ongoing continuing medical education so as to maintain their licensure/national certification and PA license.
- Independently conducts ongoing promotion to community groups that includes developing written materials and public presentations.
- Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.
