We introduced the Unified Care program to tackle chronic disease management. Our Care specialists provide support beyond the doctor's office through Chronic Care Management and Remote Patient Monitoring (RPM), aiming for improved health outcomes.

With the U.S. Food and Drug Administration Emergency Use Authorization granted to our COVID-19 Antigen Rapid Test in November 2021, we've played a pivotal role in supplying at-home COVID tests to various entities, reaffirming our commitment to enhancing people's well-being.

Summary:

We're in search of a passionate Clinical Assistant to join our Unified Care team. In this role, you'll blend technical prowess with clinical expertise to develop an innovative, team-based chronic disease management program. Utilizing devices and our Unified Care platform, you'll assist in managing patients with chronic conditions both onsite and online, collaborating closely with other clinical team members.

*This role is eligible for clinical hours, providing valuable experience for those pursuing medical school, PA programs, nursing school, etc.

Responsibilities:

Demonstrate tech-savviness and patience in assisting patients, particularly the elderly, in understanding and troubleshooting our mobile app and medical devices.
Effectively communicate technology concepts to individuals with varying levels of technical proficiency.
Collaborate seamlessly with teammates, providers, and clinic staff.
Assist with direct patient care procedures, including checking in patients, obtaining histories, taking vital signs, and preparing charts.
Maintain accurate electronic health records documentation.
Perform routine specialized procedures, such as setting up iHealth products, under established protocols.
Coordinate clinic inventories and medical supplies.
Educate and advise patients on medical issues within established parameters.
Manage patient flow following provider schedules and appointments.
Conduct patient outreach calls to schedule appointments, provide follow-up care information, and promote health education.
Qualifications and Skills:

Associate degree or above required.
Exceptional communication skills and attention to detail.
Demonstrated patience and empathy, particularly with elderly individuals.
Familiarity with medical procedures and terminologies preferred.
Strong problem-solving skills and adaptability.
Collaborative team player with leadership potential and a commitment to precise record-keeping.
Bilingual proficiency in Vietnamese required.
Proficiency in Google Workspace (Chat, Docs, Sheets, Slides) and Zoom.
