Job description
We are currently recruiting for a Licensed Physical Therapist to join an expanding Home Care team. The main office is in Laurel, and PT will cover Montgomery and Howard County. The successful candidate will play a pivotal role in serving as a collaborative partner in the journey of restoring motion for patients, ensuring optimal functionality.
Key duties involve primarily with post-operative patients in their homes to support recovery. The role entails consulting with patients to understand their current condition, conducting mobility assessments, and developing customized treatment plans. Additionally, you will design individualized fitness-oriented healthcare programs and administer physical therapy treatments to alleviate pain and enhance patient mobility. Collaborative execution of plans with a Physical Therapy Assistant (PTA) is common.
This position involves daily travel within a limited area, visiting 4-6 patients per day from Monday to Friday, between 8:00 AM and 5:00 PM (most PT sessions conclude by 3:00 PM), with the requirement to reach the initial patient appointment by 8:30 AM. This role offers an excellent work-life balance, servicing patients in their homes primarily located in Howard and Montgomery County.
Responsibilities:
· Physical Therapy Assessment and Skilled Treatments.
· Client Care Coordination. Teaching, Supervision and Counseling Clients and Caregivers.
· Documentation of Interdisciplinary Communication Supervision of Ancillary staff.
· Problem Solving and Critical Thinking Skills.
· Documentation. Ability to effectively and safely transfer patients with significant physical and/or mental impairment.
· Time Management Development of Plan of Care
· Creating personalized fitness-orientated care programs for patients
· Documenting client care and submitting notes on the same day as visit
Requirements
· Master Degree in Physical Therapy and Maryland license required
3 years clinical/ 2 years home care experience.
· Driver License and fully insured, and reliable vehicle.
· Ability to use computer and software proficiently
· Manage patients with various personalities
Drive to continually learn and grow.
Excellent interpersonal communication skills
CPR Certified
