What You Will Do: - Prioritizes activities each morning, highlighting any pertinent information necessary for the hospitalist in rendering his plan of care.
- Coordinates the treatment process of patients in conjunction with the hospitalist, staff physicians, nursing and ancillary services through utilization of appropriate resources, assisting the hospitalist in the identification of a plan of care that will enhance client outcome and reduce healthcare costs.
- Collaborates with Case Management team:
- On utilization reviews of clinical evidence to support initial admission and continued need for the acute care stay.
- In identifying resource referrals to community agencies, national organizations, medication assistance programs etc. as indicated.
- In providing guidance for discharge plan and follow-up needs.
- To ensure that education and/or written resources are provided for the patient, family and/or significant others. Encourage feedback and verification of understanding to promote health education and post-treatment care, including arranging follow-up appointment.
- Works collaboratively with the healthcare team, family and patient in assimilating psychosocial information relevant to the plan of care and transitional needs from the acute care setting. This process is supported by current research and evidence-based practice.
- Intervenes and recommends practice guidelines for the hospitalist, treatment protocols and clinical pathways as part of agency regulation requirements and core measure initiatives.
- Adheres and is sensitive to patient’s rights by involving them and their designated family member in treatment process through written and oral information as indicated.
- Maintains communication with the primary medical physician to advise of admissions and/or discharges, the physician advisor, and interacts with medical staff to facilitate appropriate utilization of services.
- Conducts collaborated, interdisciplinary discharge instructions to all physician assigned patients.
- Collaborates with the Discharge Specialist in completing post acute care needs of patient.
- Provides education to the healthcare team regarding CORE Measures.
- Participates in orientation of hospitalists, residents, and medical students.
- Assumes the duties and responsibilities of the Hospitalist Coordinator as needed.
- Participates in department performance improvement activities as assigned and attends required training.
What You Will Need: - Must be a graduate of an accredited program of professional nursing.
- Must possess current RN license from State of Ohio, BSN preferred. Three years of healthcare experience required.
- Charge Nurse experience required.
- Competent with CPOE, Medstracker, and EMR.
- Willing to work with educational programs.
- Willing to work a flexible schedule and capable of adapting to changing environment.
- Ability to work effectively with the Medical Staff.
- Must possess current CPR certification.
- Strong clinical background and demonstrated critical thinking skills.
- Demonstrated leadership skills, organizational skills, and priority setting.
- Strong interpersonal skills. Ability to understand and effectively communicate in both written and oral form and handle sensitive telephone and face-to-face inquiries.
- Knowledgeable of equipment used in pertinent clinical area, location of reference manuals, and resources for consultation.
- Knowledge of major insurance/Medicare terminology and benefits is beneficial.
