Job description
Overview
- Provide palliative care consultations for patients with serious or life-limiting illnesses and their families
- Collaborate with the interdisciplinary palliative care team, primary care providers, and specialists to develop and implement individualized care plans
- Manage symptoms, address goals of care, and facilitate advance care planning discussions
- Educate patients, families, and staff about palliative care and hospice services
- Participate in quality improvement initiatives and research activities related to palliative care
Qualifications REQUIRED QUALIFICATIONS - Master's degree or higher in nursing or physician assistant studies from an accredited program
- Current Illinois license as a nurse practitioner or physician assistant
- Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
- Minimum of two years of experience as an APP in palliative care or a related field is preferred
- Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
- Proficiency in electronic medical record (EMR) systems
