Job description

Overview

• Provide palliative care consultations for patients with serious or life-limiting illnesses and their families
• Collaborate with the interdisciplinary palliative care team, primary care providers, and specialists to develop and implement individualized care plans
• Manage symptoms, address goals of care, and facilitate advance care planning discussions
• Educate patients, families, and staff about palliative care and hospice services
• Participate in quality improvement initiatives and research activities related to palliative care

Qualifications

REQUIRED QUALIFICATIONS

• Master's degree or higher in nursing or physician assistant studies from an accredited program
• Current Illinois license as a nurse practitioner or physician assistant
• Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
• Minimum of two years of experience as an APP in palliative care or a related field is preferred
• Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
• Proficiency in electronic medical record (EMR) systems
