Mercy Community Healthcare is looking to hire a full time Patient Navigator. We are looking for energetic, self-motivated individuals, with a passion for making a difference in the lives of those we serve in our community.

We provide care to the uninsured, underinsured and insured through pediatric and adult primary care, mental and behavioral health services, and coordination of patient support services.

What Is This Job All About?
- Assist patients with their medical needs as well as addressing social determinants of health.
- Serve as a contact, advocate, and informational resource for family, community, and other social or medical agencies.
- Facilitate referrals between primary care and subspecialists.
- Assist in linking patients with services, through referrals, at other medical facilities and community agencies.
- Maintain and update all patient interactions through proper and timely documentation.
- Advocate for needed services on behalf of patients, such as assistance with medication costs or transportation barriers.
- Become familiar with community services, referral sources, and schools to assist patients.
- Treat patients with respect and dignity, portraying cultural competency in your awareness of the diverse needs of the patients served.
What We Need from You:
- Must have a High School Diploma or equivalent with at least 1 year of medical office experience preferred. Bachelor's preferred in a medical or social service field.
- Substantial interpersonal skills necessary to interact with patients, their families, and to collaborate care with other health team members.
- Must possess good written and verbal communication skills and ability to multitask and manage time appropriately.
- Possess personal values consistent with the family-centered approach to care and be sensitive in dealing with culturally and economically diverse patient populations.
- Computer skills required and include Word, Outlook, and Excel.
- Experience working with an EMR (Electronic Medical Record) is preferred.
- A basic knowledge of medical terminology and pharmaceuticals is preferred.
- Bilingual (English/Spanish) preferred.
