Job description

JOB DESCRIPTION:

The primary purpose of your job as the Home Health Aid/Recreation Aide/Floor Personnel is to provide participants with assistance in ADLs, personal grooming, toileting, and routine care under the direct supervision of the Program Director. Additionally, he/she will be involved with assisting the Activity Director with all activities, trips, and special events.

RESPONSIBILITIES:

· Provide safe, clean environment for participants.

· Provide general assistance to participants.

· Socialize with participants and make sure they socialize with others

· Takes charge and/or assist in activity/recreation program and discussion groups.

· Participate in clerical duties pertaining to day center.

DUTIES:

Report Participant absences to Program Director.

Assist Participant with clothing upon arrival and departure.

Supervises clients who smoke as needed.

Assist participants with ADL's as necessary and toilet as needed.

Assist with serving of Breakfast/Lunch/Snack and assisting participants who need help.

Initiate and/or assist with exercises daily.

Let the Program Director know of supplies that need to be ordered (kitchen/cleaning/activities).

Reports changes in participants condition to the Program Director or manager in charge. Communicates with Activity Director to plan, coordinate and implement multipurpose recreational programs.

Prepare daily craft material for individualized or group instruction.

Transport/assist participants to entertainment/special programs in facility outdoor area and on external trips and to van when dismissed.

Performs other tasks necessary for the effective operation of the program, such as ambulation when registrant is agitated and one to one communication.

Assist with recreational table games. Encourage active participation of participants.

Report changes to Program Director or Recreation Director. Keep all activity areas neat.

Promotes communication between participants and staff.

Prepare utensils for the following day, make sure all the tables are clean, turn off computers, televisions, & cable boxes

Make sure everything is clean & straighten for the next day.

Functions well as a member of the Interdisciplinary Team. Maintains confidentiality of all information regarding participants.

Attend in-services and all training given at the Center.

Maintains flexibility and displays ability to adapt to a variety of situations.

QUALIFICATIONS:

Licensed and certified by NYS Department of Health as HHA/CAN (preferred)

Food Servers License/Certificate (preferred)

CPR Certified (preferred)

Good command of English/Spanish language; ability to read and write English/Spanish.

Ability to communicate easily with participants and other staff members.

Physical stamina to provide necessary physical assistance to participants.

Ability to follow instructions and exercise initiative and creativity.

Full Time: Monday to Friday 

Hours: 9am to 5pm

ATTIRE: Neat appearance, conservative clothing, and appropriate footwear.
