Position: Referral Coordinator
Location: Hackensack, NJ
Duration: 3 Months
Job Summary:
The Referral Coordinator manages the referral process for referrals originating from HMH Physician Services Division to other physician practices or HMH Network Facilities. They ensure all initiated referrals are completed and reported back to the HMH referral source, organizing effective reporting on the status of referrals for all stakeholders.
Essential Job Functions:
Referral Process:
- Oversee the referral process for assigned providers.
- Ensure accurate data entry of demographic information, referral specialty, destination, and authorizations (if needed).
- Collaborate with providers to identify urgent versus routine referrals and prioritize workflow accordingly.
- Ensure associated orders are placed in the EMR to validate the referred service.
Scheduling Process:
- Ensure the referred service is scheduled and patients are informed of their appointment.
- Coordinate with referring and consulting offices, the Patient Access Center, and patients.
- Enter appointment status into Epic for stakeholders' awareness.
- Alert referring providers if patients cannot be seen or have not accepted the appointment.
Communication/Reporting Process:
- Follow up with patients and consulted providers to ensure appointments are kept.
- Interact with consulting services to ensure status reports or assessments are generated and forwarded to the referring provider.
- Update referral status in EMR and generate status reports as needed by stakeholders.
- Participate in performance improvement activities to address Referral Loop closure deficiencies, patient satisfaction issues, and provider satisfaction concerns.
Patient Satisfaction:
- Promote patient satisfaction through a commitment to providing excellent service.
- Communicate potential patient satisfaction issues to the manager.
- Stay informed regarding the practice’s performance on patient satisfaction surveys and work to improve overall satisfaction.
Other Duties as Assigned:
Complete all other assigned duties and projects in a timely fashion. Actively participate in all required meetings.
Requirements:
- Bachelor's degree in healthcare administration or related field preferred.
- Experience in healthcare administration, referral coordination, or related role.
- Knowledge of medical terminology, insurance procedures, and EMR systems (Epic preferred).
- Strong communication and interpersonal skills.
- Ability to prioritize tasks and work in a fast-paced environment.
