Our Practice

General Summary:
The Physician Assistant or Advanced Practice Clinician provides care management services to medically complex gastroenterology patients. They serve as an advocate for patients and their families and assist them to navigate the health care system. The Care Manager facilitates the continuity of a patient's care by functioning as a liaison between EPGI, primary care providers (PCPs), specialty care providers and inpatient care teams, as well as payers, home care providers, subacute and chronic care facilities and community resources. They collaborate with ambulatory, emergency department and inpatient care teams with the goals of optimizing communication among care providers, promoting seamless transitions between care settings and facilitating the patients' medical care.
Essential Job Responsibilities:
Education:
Experience: At least 1 year of APC experience preferred
Mental/Physical Requirements: Light to medium duty. Requires frequent lifting and carrying items weighing up to 30 pounds unassisted, including assisting patients when required. Adequate physical ability including sufficient manual dexterity to perform the requisite health care service, including injections. Requires frequent bending, reaching, repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift. Requires exposure to communicable diseases, bodily fluids, medicinal preparations and other conditions common to a clinic environment.
