Position: Registered Nurse - (Primary Care/Harbor Care Health & Wellness Center)
Department: Harbor Care Health and Wellness Center
Reports to: Director of Nursing
Pay Status: Full Time
Work Schedule: Generally, Monday through Friday; 8:00am - 5:00pm.
Location: Nashua

Job Summary: The Registered Nurse is a Professional employee, trained to provide skilled personal care and health related services. The RN functions under the direction, instruction and supervision of the Medical Director/Program Manager designee.
Requirements Basic Qualifications: - Associate degree in nursing from an accredited school of nursing; bachelor's degree preferred.
- Two (2) years of Medical/Surgical experience as a Registered Nurse.
- Current registration and licensure in the State of New Hampshire.
- Current CPR certification
Preferred Qualifications :
- Bachelor's degree is desired but not required.
- Resourcefulness, flexibility and integrity.
- Experience working in a Federally Qualified Healthcare Center organization is a plus.
- Experience in a community healthcare setting.
- Knowledge of Microsoft Office and Centricity EMR software preferred.
Knowledge and Abilities: - Good oral, written and reading communication skills. Have the ability to understand verbal and written instructions. Read and write documentation in patient record.
- Be aware of and adhere to HIPAA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients and this agency.

- Proficient in taking vital signs and basic RN skills.

Responsibilities: - Provides anticipatory guidance, screening and health education to patients to promote and maintain.
- Initiates treatment for patients with commonly occurring health problems which lie within the nurse specialist's scope of practice by using a systematic approach to history and physical appraisal of patients and/or families through clinical judgment of evaluated data. Prescribes, coordinates, delegates and evaluates.
- Monitors patients with stabilized long term or chronic illness and aids and supports individuals and their families.
- Plans appropriate care utilizing community resources and acts as a liaison by relaying information necessary for continuity of care to other health professionals and agencies.
- Instructs individual and/or families or groups about health or defined health problems.
- Participates in Quality Assurance activities.
