Job description
Join our team as a compassionate Hospice Case Manager dedicated to providing end-of-life care in Trenton, NJ. In this role, you will be entrusted with the care of patients and their families, ensuring they receive comfort, guidance, and respect during this sensitive time. Your responsibilities will include overseeing the holistic care of hospice patients, requiring strong teamwork, effective communication, and exceptional clinical skills.

Responsibilities:
- Conduct thorough assessments of patients in hospice care, including monitoring vital signs and reviewing medical histories.
- Develop personalized care plans tailored to each patient's unique needs, considering medical evidence, cultural factors, and ethical considerations.
- Administer medications and treatments following prescribed orders to ensure patient safety and well-being.
- Educate patients and their families on health conditions, treatment plans, and preventive measures.
- Collaborate with physicians and healthcare professionals to coordinate a comprehensive approach to patient care.
- Maintain accurate health records, progress notes, and care plans in compliance with regulatory standards.
- Perform clinical procedures such as IV therapy, wound care, and catheterization as needed.
- Provide emotional and psychological support to patients and families, especially during challenging times.
- Adhere to infection control protocols to minimize the risk of disease transmission.
- Stay updated on medical advancements and maintain necessary certifications.
- Contribute to quality improvement efforts by participating in audits and safety evaluations.
Qualifications:
- Must hold a valid RN license issued by the state of New Jersey.
- Current certifications in BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support) are required.
- Previous experience working in hospice care is highly preferred.
- Excellent communication, interpersonal, and teamwork abilities are essential.
- Demonstrated dedication to delivering compassionate care is a must.
- Strong attention to detail and the capacity to remain composed in stressful situations are necessary.
