Summary:
Access & Admissions Nurse:
- Provides knowledgeable intake, nursing assessment, care, and communication to assure that individuals presenting/referred for inpatient admission or crisis walk-in assessment receive compassionate care and efficient access to the services they need.
- Collects data regarding the presenting needs and health status of individuals, provides documentation and nursing assessment to attending psychiatrists/advanced practice providers/service chiefs for disposition, and works to assure that the needs of patients/clients and their families are met.
- Actively seeks to provide quality services that meet the needs of patients and clients as well as other customers - families, visitors, staff, referrers, and payers.
- Communicates in a therapeutic, respectful way with patients/clients/families at their level of understanding. Demonstrates skills to put patients at ease.
- Provides responsive intake/telephone service to referrers that reflects comprehensive knowledge of internal programs and services as well as community providers, programs, support groups and other resources.
- Completes thorough nursing assessment and medical history; assesses patient/client functional level according to professional standards.
- Reviews and documents/enters demographic data, clinical/medical status of patient/client, prescriptions and other medications, insurance and financial information.
- Determines that patient’s/client’s medical needs are an appropriate fit for the hospital and its services; refers to acute care hospitals individuals who are too medically fragile.
- Documents/communicates the patient’s/client’s presenting problems, health status, and related details to facilitate the physician’s/Nurse Practitioner’s review.
- Recognizes medical concerns and conditions requiring urgent response and initiates appropriate action.
- Responds to Code Blue and other calls for clinical/medical response.
- Promotes and maintains a safe and therapeutic environment in accordance with established standards of practice, Hospital policies and procedures, and external regulatory agency requirements. Responds effectively to patients/clients in crisis.
- Completes written documentation and accurate data entry into the hospital’s electronic systems to facilitate efficient quality care and disposition.
Requires: - Position requires RN license to practice in Maryland and at least two years of nursing experience that includes both psychiatric and med/surgery experience and demonstrates:
- Strong nursing assessment skills,
- Nursing skills sufficient to determine patients’ medical needs,
- Psychiatric nursing skills sufficient to evaluate and triage patients,
- Superb customer and patient-centered skills,
- Proficiency with electronic clinical systems for patient registration, triage, and scheduling.
- Position requires strong analytical ability and sound psychiatric nursing knowledge.
