Position Summary
Under general supervision of the Behavioral Health Director or designee, the Community Health Representative Generalist is responsible for providing safe transportation for community members to and from various health facilities and provides quality outreach in Mental Health promotion/Suicide Prevention services. The position will specifically be providing education and case management to persons by planning and implementing community-based education/prevention activities.

Essential Duties and Responsibilities
- Follow guidelines provided by the Behavioral Health Programs.
- Transport clients to and from health facilities and to attend health education.
- Provide educational materials in areas of Suicide Prevention/Mental Health Information, to all community members.
- Advocate on behalf of clients and link to available resources.
- Conduct Outreach to community members for personal care and or delivery of resources and assistance to appointments.
- Keeping updated; by memos, attending meetings, trainings, and workshops.
- Must have good oral communication skills and writing skills.
- Conducts outreach assessments and provides appropriate care to all persons with mental health needs.
- Provides mental health self-management training to individuals or group on all aspects of mental health care. Uses reliable, science-based education material or curricular for providing comprehensive s self-management training.
- Collaborates with the Behavioral Health team to develop and implement a personal care plan that meets the client's needs.
- Evaluates educational outcomes and makes appropriate referrals.
- Uses case management techniques to monitor education and clinical interventions for individual clients. Routinely monitors group data for performance improvement activities.
- Maintains confidentiality of all privileged information.
- Contributes to a team effort.
- Works with the Behavioral Health team to develop and implement community-based suicide prevention program including health fairs, Behavioral Health events, and educational activities.
- Under supervision of the Program Director organizes, evaluates and develops education materials for use by patients and family members.
- Provides comprehensive Suicide Prevention referrals and information. Makes appropriate referrals for Mental Health issues
- Obtains/ maintains continuing education in CHR education and treatment, teaching strategies and other topics related to the scope of work.
- Conducts education at all outreach events with age appropriate curriculum
- Conduct individual/community education.
- Perform other CHR duties as assigned.
Knowledge, Skills, and Abilities
- Ability to apply specialized knowledge related to the care of clients with Mental Health Issues in Santa Clara Pueblo.
- Ability to develop, implement, maintain and evaluate a comprehensive Outreach education program with an emphasis on persons with Suicidal Ideations.
- Ability to assess client's educational needs to develop individualized teaching plans
- for Suicidal Ideations. Individual must be able to provide information in a culturally sensitive approach and manner.
- Knowledge of clinical policy and procedures including but not limited to quality control measures for universal precautions, patient education standards and confidentiality issues.
- Ability to demonstrate computer skills for work processing software and obtaining Behavioral Health information off the internet.
- Must have basic knowledge of Mental Health Awareness and intervention.
- Ability to lift/ or move up to 10 pounds.
Minimum Qualifications
- High School Diploma and a minimum of two (2) years' experience working in the field of Mental Health.
- Must be able to complete Community Health Worker Certification.
- Must be First Aid/CPR certified.
- Must have a reliable source of transportation.
