Job description
Responsibilities:
- Provides psychosocial assessments to determine patient needs, makes appropriate referrals, provides counseling and supportive services to patients/families, makes referrals to the Division of Child Protection and Permanency (DCP&P)
- Identifies and maintains funding sources which supports the Family Counseling & Guidance Program.
- Maintains statistical information and analysis of data, develops outcome and measures for program evaluation, program development and facilitation of program.
Competencies:
- Maintains current knowledge of trends and advances in clinical practice and health care informatics, as well as new developments and innovations in hardware and software technology.
- Demonstrated initiative, problem identification, resolution and analytical skills are essential, as well as excellent oral and written communication skills
Minimum Education Required:
- Master’s degree in social work from an accredited school of social work required.
- LSW or LCSW required.
Minimum Experience Required:
- Minimum 5 years’ experience
- Previous experience in pediatrics preferred
- Previous experience in hematology, oncology or complex care preferred
