The North Region Hospice coverage area includes cities as north as Summitvillle and Elwood, as west as Eagle Creek, as south as just north of 38th Street in Indianapolis, and as east as Greenfield. The Hospice LPN works closely with RN Case managers to meet the needs of hospice patients and their families in their home and in long-term care facilities. The Hospice LPN demonstrates performance consistent with professional standards of practice, care, performance, the Nurse Practice Act and the mission and goals of the organization. Manages patient care in the health care continuum to achieve optimum outcomes in a safe and cost-effective manner. Hospice LPN duties include:
