Position Summary The
Nurse Practitioner is responsible for providing compassionate, evidenced-based medical care to adult patients with substance use disorders. You will develop and utilize expert assessment skills, therapeutic relationship building, specialty medication knowledge, and current best practices to help patients safely navigate withdrawal and build a strong foundation for long-term recovery from addiction. You should endorse Medication Assisted Treatment as an important treatment option for appropriate candidates.
You will provide medical services across a continuum of care that includes inpatient Medically Monitored Detox (ASAM 3.7) and High Intensity Residential Services (ASAM 3.5), as well as range of outpatient services (ASAM 2.5-1.0).
The ideal candidate has broad direct care experience, and particular expertise in addiction treatment. You should be efficient, flexible, and eager to embrace a team-based approach that models collaborative, solution-based problem solving, and authentic communication.
Duties/Responsibilities: - Admit new patients and determine the appropriate level of care based on ASAM criteria.
- Review results of Prescription Monitor Program (PMP) search.
- Reconcile current medications and determine which orders to continue, modify, or stop.
- Conduct a history & physical within 24 hours of admission.
- Diagnose Substance Use Disorders and any additional health disruptions.
- Order appropriate detox protocols or other admission order sets, and customize as necessary.
- Meet with detox patients daily to assess withdrawal and response to treatment.
- Address medical concerns as they arise.
- Educate patients about range of MAT options.
- Prescribe and adjust medications as appropriate.
- Order labs and interpret results.
- Work with each patient and the treatment team to create a plan of care.
- Respond to Emergency Codes as needed.
- Rotate on-call coverage for 24-hour coverage.
- Reconcile medications with nursing and patient prior to discharge.
- Capture all pertinent information from provider-to-provider reports when transferring patients to or from an outside facility.
- Meticulously document assessments and observations, interventions and response, and progress towards goals
- Create progress notes for each visit and consultation.
- Collaborate with multiple departments, teams, and stakeholders.
- Other duties as assigned.
Required Skills/Abilities: - Masters degree from an accredited nursing program.
- Broad clinical experience required. Addiction treatment experience preferred.
- Excellent collaborative and interpersonal skills.
- Outstanding written and verbal communication skills.
- Unrestricted licensure and board certification from ANCC or AANP
- Must have prescriptive authorization and XDEA license.
Physical Requirements: - The employee must occasionally lift and/or move up to 25 lbs.
