Pay Range: $90,000 - $122,000 annually
Job Summary
Under the direction of the Executive Director and Vice President of Clinical Services, provides for the delivery of efficient and effective nursing care within the RCFE at the Masonic Homes of California, Union City. Provides direct nursing supervision, oversight of the individual quality of care, and supports the professional development of the RCFE licensed and direct care staff. This person is a key member of the nurse management team and shares responsibility of promoting health and wellness through assessment and case management.
Essential Functions
- Provides leadership in ensuring quality nursing care to all residents. Supports and practices the philosophy, nursing objectives and standards of the Masonic Homes of California.
- Actively engages in the Wellness Program, encouraging individual residents to participate in their own wellness plan and assisting in the development of that plan.
- Is available to consult with any nurse, therapist, or physician with regard to resident needs/care and may act as a liaison to the M.D. in order to expedite quality care.
- Oversees the ID team that develops, implements and evaluates the resident service plans, which comply with physicians’ orders, government regulations, residents’ care policies and Masonic Homes mission and philosophy.
- Provides and coordinates crisis intervention and service to the residents in cases of emergency.
- Coordinates & supervises professional nursing intervention at the earliest appropriate time to hasten resident recovery to wellness and prevent hospitalizations and crisis.
- Coordinates & supervises preventative health counseling to residents whenever possible to increase overall wellness. And coordinates with the quality, active living, rehabilitation, administrative, and other interdisciplinary team members to provide the best possible quality of life and lessening risk of illness and injury.
- Actively participates in the operational budget process, identifying needs and providing input for variances.
- Originates, develops or implements new methods to accomplish objectives. Generates innovative ideas to enhance overall operations.
- Is a leader in the Care Coordination, Quality Improvement, and Safety committees. Assists in identifying problems, participates and leads, as appropriate, in the development of solutions.
- Reviews all RCFE Clinical policies and procedures annually and recommends updates to the Policy Oversight Committee. Works with the Compliance Officer to maintain relevant policies and procedures that are reviewed according to MHC policies and current regulations.
- Has direct supervisory responsibility of all RCFE nursing staff including Registered Nurses (RNs), Licensed Vocational Nurses (LVNs) and Nurses’ Aides (NAs/CNAs) in the RCFEs in Union City, as well as oversight of the Medical Transportation for Union City.
- Supervises the RCFE DSD and is responsible for insuring that Masonic Homes of California provides the RCFE staff with the required educational offerings to ensure the competency of all of the RCFE staff in the provision care and services to our residents.
- General oversight of the administration of the policies and procedures of the RCFE Clinical team including medications and treatments and evaluates the effectiveness of these medications and treatments by applying education, experience and knowledge within their scope of practice.
- Coordinates Quality, Infection Control, and Employee Health efforts with the Clinical Operations leadership in Union City.
- Participates in Facility Emergency Plan training and fire drills, and if an emergency or disaster occurs, immediately assumes the pre-assigned emergency duties.
- As the supervisor of the DSD is responsible for providing education and documenting competency of RCFE staff to ensure compliance with all legal and regulatory agencies.
- Reports directly to the Compliance Officer for all regulatory/competency/compliance responsibilities.
- Responsible for keeping the Primary Care Physician and the Clinical Operations leadership informed, orally and in writing as deemed appropriate, of any changes in residents’ condition.

Requirements
Minimum Requirements
- Current California Nursing License (LVN or RN) required.
- A minimum of 5 years of Assisted Living/RCFE nursing experience preferred.
- A minimum of 1 year nursing supervisory experience required.
- Home health care management experience and senior healthcare experience preferred.
- CPR certified.
- Ability to travel overnight within the state at least quarterly required.
- 24/7 accountability required.
- May need to work/cover on off shifts at times.

Maintains confidentiality of appropriate resident care information to assure their rights are protected.
Reports all incidents/accidents, unsafe and hazardous conditions/equipment immediately. Follows established safety rules and regulations. Maintains work area in a clean, orderly and safe manner.
