Job Overview:
As a Patient Health Advocate, you'll be at the forefront of our mission, working closely with our clinical care team to guide and support our patients through their healthcare journey. You'll have the opportunity to work in a hybrid telehealth environment, combining remote work with in-person visits to our members' homes.
Your Responsibilities:
Build strong, supportive relationships with our patients, acting as their primary advocate and trusted confidant.
Collaborate with physicians and nurse care managers to follow up on health management plans and goals.
Conduct initial assessments to determine the most suitable program for each patient.
Document your activities in our care coordination platform and ensure that care plan activities are conducted effectively.
Engage with patients over the phone and in-person, providing guidance on self-care and helping them schedule provider appointments.
Assist patients in accessing community and government-based services, including filling out paperwork and arranging transportation.
Facilitate communication between patients and various healthcare providers to ensure seamless care coordination.
Participate in integrated care team meetings and contribute to the overall success of our patient care initiatives.
Your Qualifications:
Previous experience working with Medicare, Medicaid, or special needs populations.
Strong interpersonal skills and the ability to connect with people from diverse backgrounds.
Well-connected within the community you'll serve.
Excellent written and verbal communication skills, with a demonstrated respect for cultural differences.
Willingness to travel throughout the assigned region and conduct home visits.
Empathetic, outgoing personality with a positive attitude.
Experience working in a multi-disciplinary team and engaging patients in healthy behavior changes is a plus.
