Job description
Job Description:
- Conduct assessments and aide supervisions in patients' homes
- Assess patient head to toe including medications & diagnoses and document
- Supervise home health aide/personal care aide in all duties
- Create a Plan of Care for aide to follow and orient to POC
Position Requirements: - Current and Valid NYS RN License
- Must be able to travel to patient cases within the 5 boroughs/Nassau/Suffolk
- Must carry Medical Professional Liability coverage
- Intermediate ability to use computerized systems such as Gmail, Microsoft Word, Excel, and to navigate the DOH UAS health care system
Preferred Qualifications: - Bilingual Spanish/English
- Medicaid, Medicare, Managed Care experience
- Experience in Home & Community based or Long Term Care services delivery
- Experience in the use of UAS-NY Application
- Experience working with the needs of vulnerable populations and passionate about patient care
- Excellent written communication/documentation skills
