We are seeking an organized and energetic Patient Care Manager to join our team of highly motivated individuals.

Job Duties of the Patient Care Manager:
Oversee and manage the coordination of patient care across various healthcare services and departments to ensure seamless delivery of treatment and services
Act as an advocate for patients, ensuring their needs and preferences are considered in the development and execution of care plans
Monitor and maintain high standards of patient care, adherence to protocols, and compliance with healthcare regulations
Provide leadership and guidance to the care team, including nurses, therapists, social workers, and other healthcare professionals
Foster effective communication among care team members, and patients, to ensure everyone is well-informed about the patient's care plan and progress
Conduct performance evaluations for team members, providing feedback and identifying areas for improvement or professional development
Patient Care Manager Qualifications:
Typically, 5+ years of clinical experience, with at least 2-3 years of experience in a leadership or management role
In-depth knowledge of patient care practices, healthcare regulations, and quality improvement methodologies
Strong leadership and managerial abilities to guide and motivate the care team effectively.
Excellent communication and interpersonal skills to collaborate with care team members, patients
Proficiency in critical thinking and problem-solving, especially in complex patient care situations
