Addiction and Recovery Treatment Services (ARTS) include counseling, case management, Peer Recovery Services, and remediation assistance for persons with substance use disorder.

We seek a passionate and experienced Case Manager to join our Project Link team. The ideal candidate will have a solid commitment to social justice, excellent interpersonal skills, and a proven ability to support and advocate for individuals in need.

Case Managers assist clients with their basic physical, financial, social, and personal needs. Case management services for the client include outreach, psychological and need assessments, treatment planning and service provider linkage, monitoring, advocacy, counseling, crisis intervention, daily living support, medication supervision, and similar functions.

Key Responsibilities:

• Conduct thorough assessments of clients' needs and develop individualized care plans.
• Provide ongoing client support and advocacy, ensuring access to essential services and resources.
• Collaborate with community partners, service providers, and other stakeholders to enhance the support network available to clients.
• Monitor and evaluate client progress, adjusting care plans as needed.
• Maintain accurate and up-to-date case records and documentation.
• Facilitate workshops and group sessions to address common challenges and provide education on relevant topics.
• Advocate for clients' rights and represent their interests in interactions with external agencies.

Requirements:

Bachelor's degree in Human Services (Social Work, Psychology, Sociology, or Counseling) with one (1) year of experience providing case management services to individuals with a diagnosis of substance use disorder or co-occurring diagnosis of substance use disorder and behavioral health.

The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
