DEPARTMENT: Outreach

REPORTS TO: Outreach Program Manager

CLASSIFICATION: Salary Non-Exempt, Full-time

PRIMARY FUNCTIONS (Job Summary): The Outreach Case Manager provides intensive case management to youth and young adults impacted by group violence to build positive relationships and connect them with opportunities and resources. The Case Manager will utilize culturally-specific skills to meet youth where they are at, facilitate personal and social development, create healthy and on-going support systems, and foster relationships that enable them to gain a voice, influence, and place in their community.
