Description
Responsible for coordinating the somatic care for behavioral health participants assigned to them, ensuring implementation of the treatment plan in partnership with the individual and family, conducting health promotion and education, providing referrals, and offering additional support as needed. Coordinating hospital discharged and community transition. Position requires innovation and "outside of the box" thinking to ensure client outcomes. Requirements • Graduate of an accredited school of nursing • Must maintain current Maryland License as an RN or licensure in a compact state that allows practice in Maryland • Must maintain current CPR & First Aid certification • BSN or Bachelor's degree in Health Care Related Field is preferred but not required • Prior medical case management experience highly desirable • Experience in behavioral health field a plus • Interest and proficiency in education, training, and research is desiredJob tags
