We are looking for a Weekend On Call Manager RN to join our team. This position reports to the VP Care Coordination and will provide supportive care to patient and family after normal business hours, manage and direct an interdisciplinary group consisting of Registered Nurses, Licensed Practical/Vocational Nurses, Hospice Aides and support staff to enhance the quality of hospice care. Monitors scheduling of staff visits, completing admission workflow, and reviewing HIS data.
- Maintains an accurate and updated medical record, including all Interdisciplinary notes, Plans of Care, initial and on-going assessments and any other information pertinent to the care of the patient.
- Works closely with other members of the Interdisciplinary Group to provide physical, psychosocial, educational, emotional and spiritual services including bereavement support to the patient and family to achieve the highest quality of care.
- Maintains management of each patient and acts as primary liaison between physician, patient/family and hospice team to be fully aware and abreast of all clinical issues, staffing and scheduling concerns.
- Registered Nurse with minimum of Associates Degree in Nursing required.
- Bachelors degree in Nursing (BSN) from an accredited school of nursing preferred.
- A minimum of one (1) year experience in hospice required.
- A minimum of one (1) year of management experience preferred.
- Current license to practice nursing in the state of requested employment.
- CPR Certification.
