Woodcrest Rehabilitation located in Flushing, NY is seeking
- Full time Social Worker (BSW or MSW)
- Long Term Care/ Subacute experience preferred
- Attend and participate in interdisciplinary care plan meetings
- Complete MDS while developing and maintaining individual care plans
- Interview and access resident’s needs during admission interview process
- Work with the team to coordinate discharges
- Work with resident to maintain psychosocial well-being.
