We operate with a network of over 150 therapists throughout Michigan. W ere thrilled to invite an exceptional Occupational Therap ist to join our team in a n exceptional subacute care facility. Our mission of this program is to make a positive impact on the lives of our patients through compassionate and effective rehabilitation services.

Responsibilities:

  1. Assessment and Evaluation: Conducting comprehensive assessments to evaluate the physical cognitive emotional and social aspects of geriatric patients. This involves analyzing their functional abilities identifying limitations and understanding their personal goals and preferences.

  1. Developing Treatment Plans: Collaborating with the healthcare team to create individualized treatment plans based on assessment results. Setting realistic and clientcentered goals that address the unique needs and challenges of geriatric patients.

  1. Implementing EvidenceBased Interventions: Providing direct occupational therapy interventions that may include therapeutic exercises activities and modalities aimed at improving or maintaining the patients functional independence and overall wellbeing.

  1. Cognitive Rehabilitation: Designing and implementing cognitive rehabilitation programs to address issues such as memory attention problemsolving and executive functioning in geriatric patients with cognitive impairments.

  1. Adaptive Equipment and Environmental Modifications: Recommending and facilitating the use of adaptive equipment and making environmental modifications to enhance the safety and independence of geriatric patients at home or in care facilities.

  1. Fall Prevention Programs: Developing and implementing fall prevention strategies including balance training and education on environmental hazards to reduce the risk of falls among geriatric individuals.

  1. Pain Management: Incorporating pain management techniques and interventions to address musculoskeletal or neuropathic pain commonly experienced by geriatric patients.

  1. Caregiver Education: Providing education and training to caregivers on how to support and assist geriatric patients in their daily activities ensuring a safe and supportive home environment.

  1. Collaboration with Interdisciplinary Team: Collaborating with physicians nurses social workers and other healthcare professionals to coordinate care and address the holistic needs of geriatric patients.

  1. Outcome Monitoring and Documentation: Regularly monitoring and documenting the progress of geriatric patients adjusting treatment plans as necessary and maintaining accurate records in compliance with healthcare standards and regulations.

  1. Advocacy: Advocating for the rights and wellbeing of geriatric patients ensuring that their unique needs and preferences are considered in the development of care plans and interventions.

  1. Research and Continuing Education: Staying current with advancements in occupational therapy practices participating in research activities and pursuing continuing education to enhance the quality of care provided to geriatric patients.
