Job description
Min Rate = $26.08 / Max Rate = $37.82 *Placement is in accordance with the Collective Bargaining Units Agreement.
The Family Medicine Center Licensed Practical Nurse has responsibilities that include intake, processing of patients, phlebotomy and EKGs, simple patient/family treatment as directed by the professional staff.
- Practices within the boundaries of the NYS Practice Act for Licensed Practical Nurses and provides care according to Standards of Care and Practice (generic and unit-specific). Under the supervision of a Physician.
- Provides nursing care utilizing the nursing process from young adult to geriatric and is able to:
- State age specific physiological, psychological and spiritual needs.
- Modify care given to meet age specific needs as directed by Physician.
- Understand the process of aging.
- Assists in planning a formal plan of care for patients and directly participates in the implementation of the plan under the direction of a Physician.
- Gathers information from patient to assist provider in assessing patient.
- Provides documentation as written communication.
- Provides nursing care for the patient requiring pain management.
- Provides nursing care to the patient requiring oxygen therapy.
- Provides nursing care to the patient requiring medication administration.
- Provides nursing care for the patient with a tracheostomy and/or requiring suctioning.
- Provides nursing care to the patient requiring peripheral or continuous IV therapy including IV starts.
- Provides nuring care for the patient requiring assessment and treatment with a variety of monitoring and treatment devices to include blood glucose and pulse oximetry.
- Adapts positively to changing workload by reprioritizing responsibilities in patient care assignment, goals and patient care management.
- Knowledgeable regarding the JCAHO Standards for Patient Care and other applicable standards and demonstrates compliance.
- Demonstrates the ability to utilize and access various specialized staff and resources for patient care outside of clinical experience and knowledge base.
- Demonstrates a working knowledge of Advance Directive practices.
- Attends at lease 70% of staff meetings and reviews minutes for 100% of staff meetings per year.
- Makes judicious use of supplies and patient care items to support cost containment initiatives.
- Maintains a safe environment. Assists in maintaining unit equipment and reports malfunctioning equipment to appropriate personnel. Takes corrective action to assure safety of patients/staff which may be compromised because of faulty or "down" equipment.
- Identifies ethical dilemmas and reports to supervising Physician.
- Supports hospital waste management and recycling efforts.
- Obtains and records intake information.
- Implements patient specific education programs for chronic disease, acute non-urgent health disorders and health promotion and maintenance under the direction of an Physician.
- Manages incoming telephone calls in a courteous, efficient and responsive manner, schedules appointments as directed. Gives general Family Medicine Center information and builds a positive community image.
- Identifies age specific health promotion and screening activities and coordinates their implementation.
- Administers immunizations, allergy series and other medications as required.
- Provides assistance for simple surgical procedures demonstrating knowledge of procedure specific set-ups and aseptic technique.
- Identifies and conducts appropriate intervention for suspected domestic violence and adult abuse.
- Demonstrates knowledge concerning available hospital and community resources coordinating for Family Medicine Center patients as appropriate.
- Assists in obtaining data and testing.
- Process prior authorizations for medications, enteral feedings etc. with insurance companies.
- Manages data bases, including serial labs issues, notifying provider of results
- Manage provider’s schedules to ensure adequate staffing at all time.
- Schedules requests from patients for same day appointments.
- Review daily lab/radiology and other reports and distribute to providers.
- Take calls from lab/x-ray on critical reports.
- Completes disability, out of work and other forms
- Manages Public Health and other related referrals.
- Liaison to pharmacies, nursing homes, Hospice, Public Health and other community health care resources.
- Manages prompt prescription refills. Handling all necessary processes to include prior authorizations and indigent programs.
- Other duties as assigned.
- Graduate from an accredited School of Nursing.
- Currently licensed as a Licensed Practical Nurse in New York State.
- Two (2) years previous medical office experience or primary care experience preferred.
- 1-2 years of medical/surgical experience preferred.
- Ability to function as an integral part of a team.
- Excellent telephone skills
- Must be detail oriented and organized.
- Meets requirements of physical/pre-placement screening.
As applicable, the individual has training/competency in attending to the special needs and/or behaviors appropriate to the age of the patients for which care is being provided.
