Evolve Healthcare, Inc. is seeking an experienced Public Health Clinic RN with a minimum of 1 year exp. in a public or community health setting
$45.00-$47.00/hour w/OUT - Certification in Public Health (CPH)
$48.00-$51.00/hour with- Certification in Public Health (CPH)
Mileage Paid if driving over 30 miles to clinic
Duties Include:
- May provide case management services and utilization reviews for programs including California Children Services, Child Health, and Disability Prevention Program, MCAH Outreach, Medi-Cal Targeted Case Management, Nurse Family partnership, and Indigent Health Care Program.
- Investigate and interview parents; assess and screen at-risk individuals; Act as a patient advocate.
- Instruct parents on child safety, growth, development, signs of abuse, poor nutrition, and failure to thrive.
- May provide crisis assistance and participate in patient meetings for "special needs" children.
- May work directly in immunization, STD, TB, and HIV Testing clinics.
- Administer TB meds. perform and read skin test for TB diagnosis.
- Search for unreported or missed cases of communicable diseases.
- Conduct home visits t collect and /or complete health forms related to communicable diseases.
- Provides outreach and education by giving presentations and teaching classes to the public.
- Provide orientation and training to community providers to assure adherence to quality standards.
- Acts as "on-call" nurse to receive referrals and provide information.
- May develop in-services programs for care providers, provide Targeted Case Management Services, and engage in outreach and other activities to enhance services to Medi-Cal beneficiaries
Assignment Details:
- Tentative Start Date: ASAP
- 26 week(s)/6 months or ongoing
- Shift Information: Monday - Friday 8:00 AM- 5:00 PM or 9:00 AM - 6:00 PM
Facility Requirements:
- Minimum Years of Experience: 1
- Must have a Current valid California RN license
- Must have minimum 1 year exp. in a public or community health setting
- Certification in Public Health (CPH) Preferred
- Must have BLS certifications (Must be through the American Heart Association)
- Must be able to pass drug screen and background check
