Job description
Responsibilities:
- Fairfield and New Haven counties
- Provides end of life care including Initial Assessments and treatments
- Initial assessment of patient and develop a care Plan
- Coordination of services with the patient, family, and hospice interdisciplinary team members
Requirements:
- Valid Registered Nurse (RN) license
- RN Diploma, Associates or B.S. degree in nursing
- Minimum 1 year of hospice agency experience
- Valid Driver's License, auto insurance and reliable transportation
- Excellent observation, problem solving and communication skills
- Compassion and integrity
