Job description

Interpret medication orders or prescriptions, review patient profiles, and prepare and dispense medications, drugs, and other pharmaceuticals for patient care according to professional standards and state and federal legal requirements.

Performing tasks like: Reviewing prescriptions issued by Physician, or other authorized prescriber to assure accuracy and determine formulas and ingredients needed;  Answering questions and provides information to patients or healthcare representative on drug interactions, side effects, dosage and storage of pharmaceuticals; and  Monitoring the health and progress of patients in response to drug therapy to ensure the safe and effective use of medication.  

Requirements

MINIMUM REQUIREMENTS:

• Bachelor Degree or PharmD
  
• Licensed Pharmacist by the State of Indiana
  
• One (1) year hospital experience as a Pharmacist
  
• Read, write and speak the English language as well as computer literate and mathematical ability to successfully perform the essential duties of this position.
  

HIGHLY PREFERRED:

• Three (3) years of hospital pharmacy experience
