Position Overview:
As a Home Health Licensed Practical Nurse (LPN), you will play a crucial role in delivering personalized and holistic care to patients within the comfort of their homes. Working as part of a multidisciplinary team, you will assess patient needs, administer medication, implement care plans, and provide support to enhance overall well-being.
Key Responsibilities:
- Patient Assessment:
- Conduct thorough patient assessments, including physical and psychosocial evaluations, to develop individualized care plans.
- Medication Administration:
- Administer prescribed medications and treatments according to physician orders and established protocols.
- Wound Care:
- Provide skilled nursing care, including wound care, dressing changes, and monitoring of vital signs.
- Patient Education:
- Educate patients and their families on health conditions, treatment plans, and self-care techniques to promote overall wellness.
- Care Planning:
- Collaborate with the interdisciplinary team to develop and update patient care plans based on ongoing assessments and changing needs.
- Communication:
- Maintain clear and concise communication with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
- Documentation:
- Accurately document patient assessments, interventions, and progress in accordance with regulatory standards and agency policies.
- Compliance:
- Adhere to all regulatory requirements, including maintaining confidentiality and safeguarding patient rights.
Qualifications:
- Education and Licensure:
- Graduation from an accredited practical nursing program.
- Current and valid LPN license.
- Experience:
- Minimum of 2 years of nursing experience, preferably in home health or a related setting.
- Skills:
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Proficient in the use of electronic health records (EHR) systems.
- Certifications:
- Basic Life Support (BLS) certification.
