Position: Per Diem Chaplain Location: Santa Clarita/San Fernando Valley Status: Per Diem (As needed)

Responsibilities:

1. Conduct spiritual assessments and provide emotional and spiritual support to patients and their families in accordance with their beliefs and values.
2. Collaborate with the interdisciplinary team to address the holistic needs of patients, ensuring the integration of spiritual care into the overall care plan.
3. Offer guidance and counseling to patients, families, and staff members during times of crisis, grief, and loss.
4. Facilitate religious rites, rituals, and sacraments as requested by patients and families.
5. Maintain thorough and confidential documentation of chaplaincy assessments, interventions, and interactions. **Attend IDG, every 14 days in person

Qualifications:

1. Ordination or endorsement by a recognized religious or spiritual faith group.
2. Master’s degree in divinity, theology, or a related field from an accredited institution.
3. Minimum of 1-2 years of experience in providing spiritual care, preferably in a healthcare or hospice setting.
4. Strong communication and interpersonal skills, with the ability to connect with individuals from diverse cultural and religious backgrounds.
5. Understanding of ethical considerations related to spiritual care and a commitment to maintaining confidentiality.

Requirements:

1. Availability for on-call duties as needed.
2. Valid driver's license and reliable transportation for home visits.
3. Ability to work collaboratively in a team-oriented environment.
