Position Summary: The Registered Nurse: Clinical Assessment and Eligibility determines clinical eligibility for Mass Health recipients/applicants requiring nursing facility care per regulations and promotes community service options that meet the consumer's wishes and needs for discharge. The Registered Nurse: Clinical Assessment and Eligibility works with an interdisciplinary team to assess and assist consumers requesting to transition from the nursing facility back to the community, including MassHealth and other community programs/insurance plans.

Essential Functions:

  1. Determines clinical eligibility for MassHealth recipients/applicants for nursing facility care, adult day health, home- and community-based waiver services, and other community-based programs.
  2. Completes necessary documentation requirements according to the Executive Office of Elder Affairs and MassHealth regulations.
  3. Completes Nurse of the Day (NOD) tasks.
  4. Assists in the coordination of discharge planning for elders transitioning from the nursing facility setting to the community.
  5. Performs on-site evaluation and clinical assessment of consumers in nursing facilities.
  6. Explores all available community alternatives with consumer, involved supports, nursing facility staff, care managers and options counselors to develop a service plan and which addresses the consumer's needs, values and preferences, with the goal to diminish barriers to receiving services in the community.
  7. Performs Clinical Assessment & Eligibility (CAE) activities, which includes assessing and developing a personal care plan, as indicated, as well as determination of clinical eligibility for community programs.
  8. Participates in an interdisciplinary team of Registered Nurse, Care Manager, Options Counselors and Program Managers; attends appropriate case consultations and conferences.
  9. Provides support to the Home Care RN team as directed.

Additional responsibilities:

  1. Attends appropriate agency and community meetings.
  2. Participates in agency committees as appropriate, enhancing the effectiveness and quality of the clinical component of assessed needs.
  3. Provides education on health and wellness resources to consumers and families.
  4. Performs additional tasks as directed by the Nurse Manager, including piloting, implementing, and evaluating new programs as they are developed.

Qualifications:

Must be willing and able to complete assessments on a regular basis and possess good written, verbal, interpersonal and computer skills and the ability to work in an interdisciplinary team-based environment. Bilingual ability a plus.

Hours : 37.5 per week
