Midland is hiring an APRN for our Topeka Center

We're looking for a full-time APRN to join our PACE team in Topeka. Do you enjoy making a difference and helping others? You may be just what we're looking for!

Midland Care’s Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.

For Midland Care PACE participants, PACE means a team of experienced doctors, nurses and other health care professionals getting to know the participant and working with him or her to design a plan to aid in achieving health goals.

This position is to provide primary health care to the frail elderly population, focusing on disease prevention, wellness, management of minor, chronic, acute illness, palliative, and end of life care in collaboration with the Medical Director and primary care providers as applicable. The NP provides holistic care, taking into account the needs and strengths of the whole person. The NP works in close collaboration with the entire interdisciplinary team to establish realistic goals through a participant-patient plan of care.

Employment Status: Full-Time|Monday – Friday, 8:00 a.m. to 5:00 p.m.

Pay range beginning at $92,000/year - pay based on like experience/requirements.

Qualifications

Required: Graduation from an accredited nurse practitioner program with a current Kansas license as well as passed the certification exam for your chosen specialty. Must be Medicare/Medicaid certified or be willing and able to pass the exam within the first 90 days of employment. Computer skills necessary. Experience in providing seminars, staff and community education. Must possess a valid Kansas Driver’s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standard as defined by Midland policy. Availability of reliable transportation is required for this position.

Preferred: One or more years of experience as a nurse practitioner in the setting of geriatrics preferred.
