Seeking a dedicated, intelligent, and focused Family Physician to be a part of our team. Qualifications Current Indiana License and valid CSR/DEA required. Board Certified in Family Medicine – If board eligible, must be within 4 years of residency and planning to take boards within two years. Bilingual (English and Spanish) preferred.
