The Ambulatory Pharmacy Liaison supplies medications prescribed by the patient's physician for discharge in a timely manner so that the patient can be discharged efficiently from an acute care facility and transition directly to home. The liaison assists with obtaining information necessary for prescription processing, helps prioritize workflow and facilitates delivery of completed prescriptions back to the patient prior to discharge. The liaison ensures patient medication educational needs are met.

Responsibilities

A day in the life of a Ambulatory Pharmacy Liaison at Hackensack Meridian Health includes:

1. Coordinates and facilitates patient pickup of prescriptions and/or delivery of completed prescription orders to patient care areas: Helps arrange for patient delivery or pickup of discharge prescriptions in a manner which helps facilitate timely discharge.
2. Adheres to OBRA counseling rules ensuring 100% of discharge RX program patients acknowledge offer to counsel and documents acceptance or decline of offer. Provides patients with ambulatory pharmacy contact information in the event a patient has a prescription.
3. Ensures that all medications delivered to patients are delivered correctly. Uses at least two patient identifiers: Obtains patient face sheets, allergy information and RX insurance data directly or helps determine source availability for patients choosing the service. Ensure all guests/patients receive safe care. Maintain a safety first work ethic. Report any medication errors or medication variances.
4. Establish an initial greeting to patients admitted and explain services available via the Pharmacy: Works in an efficient manner to ensure that all potential discharge prescription patients are aware of the program prior to actual prescriptions being generated by the physician.
5. Facilitates distribution of medication-related educational materials and coordinates counseling with staff pharmacists where needed: When a discharged patient accepts an offer for pharmacist counseling, works to coordinate pharmacist-patient medication counseling. If personal counseling is not requested by the patient, ensure written educational materials are distributed to the patient.
6. Obtains patient authorization and consent to process discharge prescriptions through the Meridian Ambulatory Pharmacy: Minimum expected capture rate of 50%. That is, at least 50% of qualified patients contacted by the liaison will choose to use the ambulatory pharmacy to obtain discharge prescriptions.
7. Transmits required information to ambulatory pharmacy and ensures prescription fulfillment: Provides pharmacy staff timely access to information required to dispense prescriptions for discharged patients choosing to use the service. Helps pharmacy staff triage patient needs.
8. Assists Ambulatory Pharmacy staff as needed: Exhibits team player attitude. Is able to work with and assist pharmacy staff particularly on issues related to the discharge program when needed.
9. Works with floor nurses, discharge planners, physicians, unit clerks and other teams/initiatives as required: Using I.T.- generated reports and information gathered from floor-based contacts, is able to identify and schedule initial visits to qualified patients to describe the program.
10. Other duties and/or projects as assigned.
11. Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

1. High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
2. A minimum of one year of experience in a retail pharmacy or ambulatory pharmacy setting.
3. Strong knowledge of third party programs and demonstrated ability to exceed customer expectations.
4. Demonstrates compassion and empathy, and a passion to succeed while helping others.
5. Pays attention to detail and can document outcomes.
6. Strong ability to multitask.
7. Excellent written and verbal communication skills.
8. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Education, Knowledge, Skills and Abilities Preferred:

1. A minimum of two years of experience in a retail pharmacy or ambulatory pharmacy setting.

Licenses and Certifications Required:

1. Pharmacy Technician License.

Licenses and Certifications Preferred:

1. PTCB Certified Pharmacy Technician.
