Diabetes Management - Haitian  Creole Speaking NP/ PA 
Job Location: Princeton, NJ (Onsite +Telemedicine).
Job Type: Per Diem, 1099 contract.
Pay Rate: $130/hr for onsite patient care, $100/hr for telemedicine 
Schedule: Starting in April 2024, 1-2 days/month onsite, 5-10 hours/week telemedicine, flexible.

Full Description:

VitalCheck Wellness is seeking a Nurse Practitioner or Physician Assistant to provide diabetes management programs to some of our clients in Princeton, NJ area. The NP/ PA will work directly with pre-diabetic or early-stage diabetes patients on a multi-week program to help improve their wellness and lifestyle. Our office arranges the patient appointments and does the administrative work, giving you the time and luxury to provide clinical one-on-one patient care.

Responsibilities:

• Provide general wellness checkups.
• Consult with patients to understand their health concerns.
• Advise/refer patients for follow-up care.
  
• Provide diabetes management program to patients based on our program guideline.
• Consult with patients to understand their health concerns especially around diabetes.
• Educate patients around healthy eating and lifestyle.
• Educate patients around glucose monitoring.

Requirements:

• Family Nurse Practitioner (FNP)/ (FNP-C)/ (FNP-BC)/ Acute Care Nurse Practitioner (ACNP) certified. 
• Background in internal medicine, family practice, urgent care, or emergency room.
• Minimum 3-5 years of work experience following post graduate training.
• Must have current NJ license to practice in the state of NJ. 
• Must be fluent in Creole.
• Experience with diabetes management.
  
• Preventive medicine oriented.
• Patient education driven.
• Must be proficient with computers, customer service-oriented, and able to work independently.
• Must be able to commute to Princeton, NJ.
